Carriers want evidence of how people consent. Your consent happens inside the app during onboarding, not on the public website — which is fine, but you have to document it.
Two options:
A. Screenshot method (simplest). Take a screenshot of your onboarding form showing the phone field and the consent checkbox with its disclosure text. Submit it with the application.
B. Public form method. If your Careers page or a contact form on the website collects phone numbers, that form must have:
